Free school-required vaccines will be offered through the school clinic by nurses from Nicklaus Children’s Hospital. To ensure your child can receive vaccines, parents/guardians must complete and return the consent packet. Be sure to fill out all sections clearly, sign the consent form, complete the health screening checklist, and initial only the vaccines you approve. Students without a completed and signed form will not be vaccinated.
